remove sub-channels in favour of plain channels

As noted in #2, the use of sub-channels for selecting (internal, boundary) parts of the volume mesh, via the sub-channel names mesh or patches is probably too convoluted in practical terms.

Consider removal and replacement with switches:

internal  true;
boundary  true;
