Exception: applyWallFunctionBoundaryConditions.C cannot split #include
directives.

Creates linear slope only from low and high elements around the sample
value, so rate will be discontinuous.  Would be better with higher
order gradient calculation.

STYLE: use findIndices instead of findAll for coordinateSystems
