simpleFoam does not support dynamicMeshDict

simpleFoam has no way to use the run-time selection mechanism for meshes.

It only does the minimum - no flux updating for mesh motion etc.

Needs update for tutorial with following replacements:

• Allrun at this location
• dynamicMeshDict at this location
