ENH: robustness and functionality improvements for VTK output

- PstreamBuffers for parallel VTK output.
  - avoids MPI overflows for larger meshes

- new vtk::lineWriter for writing edges

- vtk::fileWriter::writeUniform now also supports processor-specific
  uniform values instead of assuming that they are identical everywhere.
