@Prashant - ready for some more testing.

@andy, @Mattijs - was thinking that we should also adjust the file index to use the OpenFOAM time::timeIndex instead of a linear index. This would align it better with ensightWrite and possibly make it easier to mix in case files. Opinions?

I'd prefer a linear index for readability on disk, e.g. when running in transient mode with a variable time step the timeIndex does not reflect the current time - but think we need to stick to timeIndex so that we can perform run continuations/easily join datasets etc.
